Job Summary: These functions/duties include but are not necessarily limited to the following: explains hospice services to patients and families and assist with the signing of consent documents. Coordinate, collaborate and develop plan of care with patient, significant others, and members of the interdisciplinary team to focus and assure patient centered outcomes. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.
