Key Responsibilities Perform comprehensive evaluations and develop individualized treatment plans based on physician referrals and resident needs. Provide skilled physical therapy services to improve strength, balance, mobility, endurance, and overall functional independence. Reassess resident progress and modify treatment plans to maximize outcomes. Supervise Physical Therapist Assistants (PTAs), rehabilitation technicians, and therapy students as appropriate. Complete accurate and timely documentation in accordance with facility policies, Medicare guidelines, and regulatory requirements. Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary care team. Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, and home exercise programs. Participate in discharge planning, care conferences, and quality improvement initiatives. Provide coverage at nearby affiliated facilities as needed to support resident care. Qualifications Graduate of an accredited Physical Therapy program. Current North Carolina Physical Therapist license (or eligibility for licensure). Experience in skilled nursing, rehabilitation, or long-term care is preferred but not required. Strong clinical judgment, communication, and interpersonal skills.
