This hybrid position combines nursing care with proactive communication and care coordination to help keep patients, families, facility staff, and the Mountain Valley hospice team informed and connected. What You'll Do: Coordinate Care: Round routinely in SNFs and ALFs, build relationships with facility staff, and participate in facility care plan meetings as appropriate. Keep Families Connected: Provide proactive communication and follow-up with patients and families, particularly following changes in condition or significant events. Provide Nursing Support: Complete PRN and supplemental LPN visits, reinforce hospice education, and recognize and communicate changes in patient condition to the RN Case Manager. Support the Care Team: Identify communication or service concerns and ensure they are communicated to the appropriate clinical team member or leader for follow-up. Prevents creation of duplicate medical records that can cause treatment safety issues and billing problems. Follows and ensures compliance with the mandate of the organization’s accrediting bodies to use identifiers to positively identify a patient prior to the delivery of patient care to ensure patient safety.
